There are three medical plan options for eligible CHC employees, administered by Horizon: the OMNIA 65/55 Plan 950, the OMNIA HSA 80/50 Plan 929, which is a Qualified High Deductible Health Plan (HDHP) with a Health Savings Account (HSA), or the OMNIA 100/50 Plan 934.

 

All three plans offer In-Network coverage only. With OMNIA, you have the option of choosing from Tier 1 and Tier 2 providers, all of which are in-network with Horizon BCBS of NJ. When accessing care at Tier 1 providers and facilities, you will have richer benefits and much lower out-of-pocket costs.
