Your Life, AD&D and Disability Benefits

Administered by Unum

VNA provides all of its eligible employees Basic Life and Accidental Death & Dismemberment (AD&D) insurance through Unum at no cost to you. Life and Disability eligibility and benefits are dependent on class.

 

Staff members who are part of a union should refer to their respective collective bargaining agreement or plan certificate for details.

Whole Life Insurance with a Long-Term Care Rider

Whole Life Insurance offers protection beyond an individual’s working years, potentially for your lifetime. With a guaranteed death benefit that will never decrease, level premiums that will never increase, cash value accumulation, living benefits, and other options, Whole Life goes beyond typical term life insurance.

 

The Long-Term care component may provide benefits if you are chronically ill and receiving qualified long term care services.

Benefit Amounts:​

​

  • Employee: Increments of $10,000, up to $200,000

​

  • Spouse: Increments of $5,000, up to $50,000

​

  • Child: Increments of $5,000, up to $25,000

​

Guaranteed Issue​

​

  • Employee: Ages 15–50: $130,000; Ages 51–80: $60,000

​

  • Spouse: Ages 15–50: $30,000; Ages 51–80: $15,000

​

  • Child: $25,000

 

To enroll in this new benefit, follow these steps:

  • Click Enroll today.

​

  • Register as a New User.

​

  • Enter the Company Identifier when prompted: VNAHG.

​

  • Complete the process.

​

Life and AD&D

VNA provides all eligible employees Basic Life and AD&D coverage through Unum, at no cost to the employee. Life and Disability eligibility and benefits are dependent on class. Staff members who are part of a union should refer to their respective collective bargaining agreement or plan certificate for details.

Long-Term Disability

VNA covers the cost of Long-Term Disability insurance for employees working 21 hours or more per week. This coverage is provided through Unum – at no cost to you.

 

The plan benefit is 60% of your basic monthly earnings up to a maximum of $10,000 per month. These benefits begin after a 180-day waiting period.

 

Note: You may not be eligible for benefits if you have received treatment for a condition within three months prior to your effective date under this policy until you have been covered under the policy for 12 months.

This benefit is for employees working and residing in New Jersey.

Group Pension Life

In addition to basic life and AD&D insurance, VNA provides employees who are eligible for and participate in the Company’s 401(a) pension plan, who work 15 hours or more per week, group pension life insurance in the amount of 1x annual earnings up to $200,000.

Voluntary Life and Dependent Life

You can purchase additional Life Coverage beyond what VNA provides:

  • Voluntary Employee Life: Choose from $10,000 to $500,000 in $10,000 increments up to 5 times your earnings. Employee must be enrolled to enroll dependents.

​

  • Optional Dependent Life for spouse: Get up to $250,000 of coverage in $5,000 increments, if eligible. Spouse coverage cannot exceed 100% of the coverage amount you purchase for yourself.

​

  • Optional Dependent Life children: Get up to $10,000 of coverage in $2,000 increments, if eligible. One policy covers all of your children until their 26th birthday. The maximum benefit for children live birth to 6 months is $250.

 

Reminders:

 

  • Coverage requests over amounts not already approved by Unum must complete Evidence of Insurability (EOI).

​

  • Once approved by Unum, payroll deductions will be updated accordingly.

​

  • You must be enrolled in voluntary life coverage in order for your spouse, and/or eligible dependent children to enroll.

Voluntary Disability Benefits

Employees have the ability to purchase Voluntary Short-Term Disability coverage and Voluntary Long-Term Disability coverage. 


Short-Term Disability is a type of disability insurance coverage that can help you remain financially stable should you become injured or ill and cannot work.

Long-Term Disability is intended to protect your income for a long duration after you have depleted Short-Term Disability or sick leave. The benefit kicks in after a 90 day elimination period.
